You are registered with RAMQ automatically at 65. From July 1, 2026 to June 30, 2027, you pay a $21.25 monthly deductible and 30% of the rest, up to $105.25 a month and $1,263 a year. A yearly premium of $0 to $789 per person, based on family income, is paid with your Quebec tax return.

Fair PharmaCare is not automatic. Your deductible and family maximum are based on your family's net income from two years ago. After the deductible, PharmaCare pays 70% of eligible costs, or 75% if someone in your family was born before 1940. After the family maximum, it pays 100% for the rest of the year.
